No account required. Your collection and the free care tools work immediately after install. An account is only for optional sync.
Photos stay yours. A photo leaves your iPhone only after you explicitly consent, and it is downsized and re-encoded first — EXIF and GPS metadata are stripped.

Subscriptions automatically renew unless cancelled at least 24 hours before the end of the current period. Your Apple Account is charged for renewal within 24 hours prior to the end of the current period. Manage or cancel in iPhone Settings → [your name] → Subscriptions.
